Failure of PFNA: helical blade perforation and tip-apex distance.

چکیده
Since the clinical introduction of proximal femoral nail antirotation (PFNA) by AO group in 2004 and PFNA-II (Asian version) in 2008, this type of cephalomedullary nail with single head-neck helical blade were commonly used in unstable pertrochanteric and intertrochanteric fractures in geriatric patients. Recently, severe complications of helical blade perforation through the femoral head, into the hip joint or even the pelvis, were reported in several case reports. After review of the PFNA literature in English, we found a total of 12 cases helical blade protrusion in 6 papers (Table 1). From the provided data and radiographs, we can summarise that most patients were old women, the fracture reduction and caput-collum-diaphyseal angle were acceptable, and most patients took full weight bearing immediately after operation. Four perforations occurred after a fall on the affected hip, one with subclinical low-grade infection, one with helical blade tail abutting the lateral cortex, and six had none aetiology. Most perforations occurred within 6 weeks. Furthermore, blade tip-apex distance (TAD) in four available patients was less than 20 mm or even 15 mm. Contrast to femoral head lag screws (blunt head, wrench-in, large axial contact area and move cancellous bone), the helical blade has its own unique characteristics in insertion mode and morphological design (relative sharp head, hammer-in, large circumference contact area and compact cancellous bone). Should the tip-apex distance (TAD) rule be modified for the proximal femoral nail antirotation (PFNA)? A retrospective study
BACKGROUND Unstable proximal femoral fractures are common and challenging for the orthopaedic surgeon. Often, these are treated with intramedullary nails. The most common mode of failure of any device to treat these fractures is cut-out. The Synthes proximal femoral nail antirotation (PFNA) is unique because it is the only proximal femoral intramedullary nail which employs a helical blade in li...
متن کامل[Results and complications of pertrochanteric hip fractures using an intramedullary nail with a helical blade (proximal femoral nail antirotation) in 200 patients].
OBJECTIVES Evaluation of the surgical management, outcome and complications in patients with pertrochanteric fractures treated with PFNA nail. MATERIAL AND METHOD A retrospective study was conducted on 200 patients treated consecutively between April 2010 and February 2012. Radiological assessments were performed before and after the surgery, and during the follow-up (fracture reduction, blad...
متن کاملReason and treatment of failure of proximal femoral nail antirotation internal fixation for femoral intertrochanteric fractures of senile patients.
The cause of postoperative failure after the treatment of femoral intertrochanteric fracture with proximal femoral nail antirotation (PFNA) was analyzed, and the reoperative methods were examined. Nine failures of 308 femoral intertrochanteric fracture patients with PFNA were treated with femoral prosthesis total hip replacement (THR) and reoperative internal fixation. All nine patients were an...
متن کاملInfluence of different great trochanteric entry points on the outcome of intertrochanteric fractures: a retrospective cohort study
BACKGROUND The Proximal Femoral Nail Antirotation (PFNA) system for treatment of intertrochanteric fractures is currently widely applied worldwide. However, even though the PFNA has produced good clinical outcomes, a poor introduction technique with an inappropriate entry point can cause surgical complications. Some researchers suggest improving clinical outcomes by modifying the entry point, b...
متن کاملOrthogeriatrics Current Awareness Newsletter DECEMBER 2015
Purpose: To investigate whether helical blade implant systems have advantages in terms of tip apex distance (TAD) and cut-out rate in comparison to conventional lag screws for intertrochanteric fractures in a geriatric population. Methods: Relevant articles were sourced from the MEDLINE, Embase, Ovid and Cochrane Library databases from inception through March 2015. All randomized controlled tri...
